Bring the List

When completing a medical history form during a routine doctor’s visit, do you struggle to remember the names of the medications you are using, the dosage, and how often the pills should be taken? Now imagine if you were asked for this important information during a medical emergency?

“It is common for many people to be under the care of more than one physician specialist, a trend that is particularly prevalent in our elderly population,” said Walter Miller, MD, a family practice specialist and President of the Medical Staff at SOCH. “This means that they may be taking a number of medications, prescribed by different doctors. Physicians and nurses need to know what medications their patients are taking, especially in the event of an emergency, so that they can effectively diagnose and treat the illness or injury.”

Now, thanks to an initiative at SOCH called “Bring the List”, it’s easier than ever for people to keep track of their medications. Through “Bring the List”, SOCH has made available to the community a free personal medication record. This simple, easy to use card allows you to list all of your medications, what the drug is being used to treat, how much you take and how often. Fill it out completely and keep it in your wallet or purse, ready to show your healthcare provider when you visit.

“Let your loved ones know where you keep this list in the event of an emergency,” suggested Dr. Miller, “and remember to add new medications and cross out those you are no longer taking so that your list stays up to date.”

Dr. Miller recommends family members who are taking care of a loved one also use the list to help keep track of their medication usage. It is also important to include over-the-counter drugs and vitamins on the list, as they can also interact with prescription medicines.

Residents can pick up a copy of the “Bring the List” brochure, which includes the Personal Medication Record, at the information desk in the lobby of SOCH, at the Family Resource Center, located on the 4th floor of the Ocean Club on Route 9 in Stafford Township, or via SOCH’s website at www.soch.org.
