Coordinated Care Throughout your Stay and Following Discharge

As a patient at Southern Ocean County Hospital, you will benefit from the services of a Case Manager and/or Social Worker. Your case manager will work with your physician and your healthcare team in developing an individualized plan of care. The case manager will help you make sure that you receive tests and procedures in a timely manner and provide ongoing education to you and your family about the care you will need. The case manager will also help you and your family with insurance issues.
